Clinical Experience in Treatment of Carotid-Cavernous Fistura

Abstract
Review of the literature shows many methods of treatment for carotid-cavornous fistula. It has been agreed that this type of lession presents many problems characterized by success or failure as well as unexpected serious complications.
The Authors report 4 cases of carotid-cavernous fistula treated in the Younsei University Hospital during the past 4 years. All except for the 3rd case, had a history of trauma as the most likely precipitating factor. Various methods including, digital compression, ligation of internal carotid artery, trapping and plus muscle implantation were applied. The elderly lady with marked arteriosclerosis and hypertension who received digital compression, expired 6 months later. Ligation of the internal carotid brought incomplete improvement in case 3. Both trapping, and the trapping procedure followed by muscle implantation into the carotid artery resulted in cure in case 2 and 4.
From our limited experience it may be wise to avoid any conclusions, but the trapping procedure supplemented by muscle implantation appears to be the best treatment we have at the present time.
